Please complete the following to apply for a B2B account:

• Download the appropriate tax exemption certificate:
  • Uniform Sales & Use Tax Exemption/Resale Certificate
  • Form ST-105 General Sales Tax Exemption Certificate (Indiana Only)
• Fill out your certificate and email it to sales@apexinds.com
• Complete and submit the form below

• No internet sales (ie. Amazon/Facebook) allowed
• We do NOT dropship our products for wholesalers
